If you do not have a current, valid photo driver’s license issued more than six (6) months ago, you must provide additional documentation to establish your identity by following the steps below. Even if your driver’s license was renewed in the same state within the last six months, you are still required to follow these instructions.
Failure to provide the required identification may result in delays in the processing of your passport application. If the government determines that the identification submitted is insufficient, your passport application may be delayed or denied. All identification issues for passport applicants are reviewed on a case-by-case basis by the U.S. government. Even if you follow the guidelines outlined here, the passport agency may still request additional identification documents before issuing a passport.
You are required to submit a minimum of five (5) documents that help establish your identity. These documents should generally be at least five (5) years old. Below is a suggested list of documents that have proven helpful for passport applicants in the past; school records, in particular, have been very effective.
You may also submit copies of other personal documents not listed here, and they will be considered. Expired documents—such as old identification cards or driver’s licenses—may also be submitted and reviewed.
Professional employee identification card
Government employee identification card (Federal, State, Municipal)
Student identification card (high school or college)
Driver’s license or state ID card
High school yearbook cover and photo pages (copies)
Selective service registration/classification card
Report of separation from active duty (DD Form 214)
Official military identification card (copy front and back)

Baptismal, confirmation and communion certificate
