All first-time or new applicants must have their applications authorized by a local government official known as a Passport Acceptance Agent (AA).
Steps:
Bring all required documents listed above to a Passport Acceptance Facility.
Inform the agent that you are using an expediting service to “hand carry” your application.
Present the following items to the Acceptance Agent for approval:
Completed DS-11 application form
Two passport photos
Proof of citizenship
Two copies of Authorization Letter
Proof of departure
Payment to “U.S. Dept. of State”
Parents sign the application form in the presence of the Acceptance Agent, who will witness your signature and place the items listed above into a sealed envelope.
Important:
Do not open the sealed envelope; doing so will invalidate the application.
Ensure the Acceptance Agent returns the sealed envelope to you.
If the agent is unsure about releasing it, refer them to page 31 or 44 of the Passport Agent’s Manual under “Hand-carrying of Executed Applications.”
Send a printed copy of your Order Form along with the required documents to the address listed at the top of your order confirmation. Include:
