Please Choose New U.S. Passport Service if:
- You never had a passport and applying for a U.S. passport for the first time.
- Your expired US passport is not in your possession.
- Your previous US passport has expired and was issued more than 15 years ago.
- Your previous US passport was issued when you were under 16 years.
Step 1 Gather U.S. Passport Application Documents
1. Fill out Form DS-11 (Application for a U.S. Passport).
Important rules:
- Complete the form online
- Do NOT sign the form yet
- The form must be signed in front of the passport acceptance agent

2. Gather Proof of U.S. Citizenship

3. Passport Photo
Submit one passport photo of the child that meets official requirements:
- Color photo
- White background
- Taken within last 6 months
- No hats or filters
- Eyes open and facing forward

Infant photos are allowed but must still meet size and background rules.
4. Proof of Identification
- State issued driver’s license (at least 6 months old) bearing a photo
- Military I. D. bearing a photo

5. Proof of Departure
Please provide one of the following,

A copy of your flight ticket, or
-
A copy of your travel itinerary issued by your airline or travel agency.
6. Letter of Authorization
Complete and print out two copies of Authorization Letter. This letter will authorize our company to submit and obtain the new passport on your behalf.
7. Processing Time & Fees
A. Government Fees (Paid to the U.S. Department of State):
Prepare a check or money order payable to “U.S. Department of State” for the government passport fee.

Starter or temporary checks are not accepted
You will also need a separate check or money order for $35 made payable to the acceptance facility where you submit the application (such as the Post Office or County Clerk/Courthouse).
B. Processing Time & Our Service Fee:

8. Place Order
Click here to complete the order form in full. You will be prompted to enter your personal details, order information, and payment information. Once everything is entered, click “Submit Order.” Your completed order form will be generated — please print it and sign where required.
Step 2 Get Your Application Authorized
All first-time applicants and anyone submitting a new passport application must have their paperwork officially reviewed and sealed by a local government representative known as a Passport Acceptance Agent (AA). Bring all required documents listed above to a Passport Acceptance Facility and inform the agent that you are using an expediting service and will be hand-carrying the application.
At the facility:

The Acceptance Agent will review your documents

You must sign the DS-11 application in front of the agent

The agent will witness your signature and validate the application
-
The agent will place the required documents into an official sealed envelope
The sealed envelope will include:

Completed DS-11 application form
-
Two passport photos
-
Proof of U.S. citizenship
-
Two copies of the authorization letter
-
Proof of travel or departure
-
Check or money order payable to “U.S. Department of State”
Do not open this sealed envelope. Opening it will void the authorization and you will need to repeat the process. Important: Make sure the Acceptance Agent gives the sealed envelope back to you. If the agent is unsure about releasing it, ask them to review the section titled “Hand-Carrying of Executed Applications” in the Passport Agent Manual (listed under the acceptance procedures section).
Step 3 Ship Documents
Send your completed document package to the address shown at the top of your order confirmation.
Your shipment must include:

The sealed envelope provided by the Passport Acceptance Agent (do not open it)

A printed copy of your Order Form

One copy of the Authorization Letter (the other two copies should already be inside the sealed envelope)
