Step 1 Prepare Passport Application Documents

1. Completed Passport Renewal Application DS-82 form online (No handwritten form )

Please click the link at the bottom of this section to complete and create your passport renewal application (Form DS-82) online. After completing the form, scroll to the bottom of the page and click the “Create Form” button. Your completed DS-82 application will then appear on your screen.
Important: The passport agency does not accept handwritten application forms. All applications must be created online.

Once the form is completed:

• Print one (1) copy of the DS-82 application

• Do not print the form double-sided

• Sign the completed application form (DS-82)

2. Your most recent U.S. Passport

• You must submit your most recent U.S. passport book.

• If you are renewing both a passport book and passport card, you must submit both your most recent passport book and passport card.

• If your passport card has been lost or misplaced, you may renew only the passport book. The passport card cannot be renewed without the original card.

• Your most recent passport must have been issued within the last 15 years.

• Your most recent passport must have been issued when you were age 16 or older.

• Your most recent passport must be undamaged.

3. Two identical passport photographs

The photo must have been taken within the past six (6) months and must accurately reflect your current appearance. It must be an official passport-style photo, approximately 2 inches x 2 inches, with a plain, clear background and printed in color.
Hats, glasses, or any items that obscure the face are not permitted. The photo must be taken facing forward; side angles or profile photos will not be accepted. Homemade or digitally created photographs are not acceptable.

4. Proof of Name Change

If the name you currently use is different from the name shown on your previous passport, you must submit an original marriage certificate, divorce decree, or court order documenting the legal name change. The name change document must bear the official seal of the issuing authority.
Uncertified copies or notarized documents cannot be accepted. All original documents submitted will be returned to you with your passport.

5. Proof of Departure

Please provide one of the following items if you have one:
-A copy of your air plane ticket or
– A copy of your itinerary from the travel agent or airline.

6. Letter of Authorization

Click here to complete and print two (2) copies of the Authorization Letter. This letter authorizes our company to submit your passport application and obtain the passport on your behalf.

7. Processing Time & Fees

A. U.S. Government Fee for Passport Renewal
Submit one (1) check or money order payable to “U.S. Department of State” (government processing fee).

• Passport Book: $212.05

• Passport Book & Passport Card: $242.05

Please note:

• Write the applicant’s full name and date of birth on the check or money order

• All personal checks must include a printed address

• Do not cross out or alter any information on the check

• Temporary or starter checks are not accepted

8. Place Order

Click here to complete the order form. You will be asked to enter your personal information, order details, and payment information. Once all required fields are completed, click “Submit Order” to generate your completed order form.

Step 2 Ship Documents

Please send a printed copy of your Order Form along with all required documents to the address listed at the top of your order confirmation.

Required Documents:

• Passport renewal application (Form DS-82)

• Your most recent passport

• Two passport photos

• Name change document (if applicable)

• Proof of departure

• Letter of authorization (two copies)

• Order Form

• Payment to “U.S. Department of State”
